Start an EOTAS Referral

Use this form to start an EOTAS referral for a child or young person who may need creative, person-centred support through BHVA.

This referral helps us understand their needs, EHCP outcomes, current situation, interests and the kind of support that may help them take their next step.

You do not need to have all the answers before you begin.

Share what you can, and we ill use the information to review whether BHVA could be the right fit.

For parents, carers, EHCP caseworkers, schools and professionals.

Before you start the EOTAS referral

The form will ask for information about the child or young person, their current situation, and the support they may need.

If they have an EHCP, please have this ready to upload if possible.

It is also helpful to share any relevant safeguarding, risk, medical or professional information. This helps us understand what needs to be in place before provision can begin.

Child or young person details

Name, age, current education status, EHCP information and local authority details.

Needs and outcomes

Current barriers, EHCP outcomes, interests, strengths and what progress could look like.

Safety and Support

Safeguarding, risk, medical information and anything we need to know before sessions begin.

This information is not about judging the child or young person. It helps us plan safely, carefully and around their needs.

What happens after you submit the referral?

Once we receive the referral, we will review the information carefully before confirming next steps.

We review the referral

We look at the child or young person's needs, EHCP outcomes, interests and current situation.

We may arrange a conversation

If needed, we will arrange a stability conversation with the parent/carer, young person or professional involved.

We consider provision

We consider whether BHVA is the right fit and what creative support may be suitable.

We write a proposal

If BHVA is suitable, we write a bespoke proposal outlining the suggested provision, outcomes and support.

Provision is subject to suitability, safeguarding, risk assessment, staffing availability and agreement from the relevant parties.

EOTAS referral form

Please complete as much of the form as you can.

If something is not known yet, you can say that in the form. The more detail you can share, the easier it is for us to understand the child or young person's needs and consider suitable next steps.

Referrer details

Please tell us who is making the referral and who we should contact about next steps.

Young person details

This helps us understand the young person's age, setting and current context.

Current situation

Please tell us why EOTAS is being explored and what support may be needed.

SEND, EHCP and outcomes

This helps us understand how support may need to link to agreed outcomes and needs.

Provision being explored

Please tell us what kind of support you are hoping to explore. It is okay if you are unsure.

Safeguarding, risk and support needs

Consent and confirmation

Please confirm that you have permission and authority to share this information before submitting the referral.

Not ready to start a referral yet?

If you are not sure whether BHVA is the right fit, you can book a suitability conversation first.

This gives us a chance to understand your situation, answer initial questions and talk through whether an EOTAS referral is the right next step.

We start by understanding

Every child or young person arrives with a different story, different needs and different starting points.

The referral form helps us understand what was happened so far, what support may be needed, and whether BHVA could offer the right creative provision.

You do not need to make everything sound perfect.

You just need to help us understand.
