Use this form to start an EOTAS referral for a child or young person who may need creative, person-centred support through BHVA.
This referral helps us understand their needs, EHCP outcomes, current situation, interests and the kind of support that may help them take their next step.
You do not need to have all the answers before you begin.
Share what you can, and we ill use the information to review whether BHVA could be the right fit.
For parents, carers, EHCP caseworkers, schools and professionals.
The form will ask for information about the child or young person, their current situation, and the support they may need.
If they have an EHCP, please have this ready to upload if possible.
It is also helpful to share any relevant safeguarding, risk, medical or professional information. This helps us understand what needs to be in place before provision can begin.

Name, age, current education status, EHCP information and local authority details.

Current barriers, EHCP outcomes, interests, strengths and what progress could look like.

Safeguarding, risk, medical information and anything we need to know before sessions begin.
This information is not about judging the child or young person. It helps us plan safely, carefully and around their needs.
Once we receive the referral, we will review the information carefully before confirming next steps.

We look at the child or young person's needs, EHCP outcomes, interests and current situation.

If needed, we will arrange a stability conversation with the parent/carer, young person or professional involved.

We consider whether BHVA is the right fit and what creative support may be suitable.

If BHVA is suitable, we write a bespoke proposal outlining the suggested provision, outcomes and support.
Provision is subject to suitability, safeguarding, risk assessment, staffing availability and agreement from the relevant parties.
Please complete as much of the form as you can.
If something is not known yet, you can say that in the form. The more detail you can share, the easier it is for us to understand the child or young person's needs and consider suitable next steps.
If you are not sure whether BHVA is the right fit, you can book a suitability conversation first.
This gives us a chance to understand your situation, answer initial questions and talk through whether an EOTAS referral is the right next step.
Every child or young person arrives with a different story, different needs and different starting points.
The referral form helps us understand what was happened so far, what support may be needed, and whether BHVA could offer the right creative provision.
You do not need to make everything sound perfect.
You just need to help us understand.
